If you want to use the LDS Church pouch, it goes like this:

Salt Lake City, UT 84130-0150
(The only thing is that with the pouch you cannot use envelopes. You write your letter on a sheet of paper, then fold it 3 times and tape the top. Then just address it like an envelope)
